Membership Benefits
Two (2) comprehensive dental exams per year
Two (2) professional cleanings per year
All necessary x-rays
One (1) fluoride treatment per year (no age limit)
One (1) emergency exam per year
15% off most additional treatments, including periodontal, restorative, oral surgery, and prosthodontic services**
Annual Pricing
Adult (14+) – $396/year
Adult + Spouse – $749/year
Child (13 & under) – $299/year
Membership Terms
Full payment is due at the time of enrollment.
Benefits become active once payment is received in full.
Payment methods accepted: credit card (Visa, Mastercard, Discover, American Express), cash, or check.
Membership is valid for one year from the date of enrollment.
All treatment fees (after applicable discounts) are due at the time of service.
Exclusions & Limitations
This is not a dental insurance plan and cannot be combined with any insurance or third-party benefits.
Services rendered by specialists outside our office are not covered.
Plans are non-refundable once used. A full refund is available within 30 days of purchase if no benefits have been utilized.
Unused benefits do not roll over to the next year.
Patients must remain active members to receive discounted rates on treatment.
The following services are excluded from discounted pricing:**
Orthodontic services (CDT Code D8000–D8999)
In-house teeth whitening treatments
